Welcome to the The BSI Project Book submission form. We're excited about this new project and even more excited to read your work. We welcome your diabetes stories and poems, written in English, from all over the world. These stories can be written from the perspective of someone living with diabetes, guardians, or healthcare workers.

Before you begin, please visit our Guidelines page, which will answer many of your questions. Following our guidelines will give you the best possible chance of being accepted.

Please complete all fields in the form below. If you live outside the United States please fill in the parts of the form that pertain to you and where you live. Even if you are writing a second entry, we need your complete information again. If you have changed your contact information after completing your entry, please let us know your new information by emailing shialearns@gmail.com.
